| Q |
I am a new patient. What should I bring
for my first visit? |
| A |
Print the new patient form from our Web site.
When you answer the questions on this form, you will gather all the
information necessary. |
| |
|
| Q |
If I am a new patient, do you need my
old medical records? |
| A |
It is helpful if you can obtain your old medical
records before your office visit. This will provide Dr. Kedainis with
more detailed history and medical information and prevent the
duplication of ordering tests that were recently completed. |

|
| Q |
What kind of information should I bring
with me to my first visit? |
| A |
Please bring your insurance cards, driver’s
license and any pertinent medical records or information. We encourage
you to review the medical questionnaire and forms that are available on
our Web site’s Patient Forms page. That way you can create the
list of the medical problems before you get to our office. This gives
you more time to think and not forget the important questions you want
to ask your physician.
